Transfer/Rollover Form Instructions

A transfer/direct rollover occurs when you authorize your current IRA custodian/trustee to transfer assets from your existing account directly to your Solo 401k Trust account. You may only transfer cash and acceptable assets to your account. Therefore, any non-acceptable assets at your existing custodian/trustee must be liquidated and delivered in cash. This form authorizes a direct transfer of cash and acceptable assets from your current custodian/trustee to into your account.

DO NOT COMPLETE THIS FORM IF:

You intend to transfer a 401k, 457, 403b, governmental or other tax‐exempt employer/organization retirement plan (i.e. Profit Sharing Plans, Defined Benefit Plans, etc.) Employer-sponsored plan usually will require you to complete their own Transfer/Rollover Request Form.  You MUST contact your Plan Administrator/Custodian or Trustee and inquire about procedure and required forms.  However, you still should review these Instructions to get yourself familiar with the process.

ONCE THE FORM IS COMPLETED, YOU MUST MAIL THE ORIGINAL FORM WITH YOUR ORIGINAL INK SIGNATURE TO YOUR CURRENT IRA CUSTODIAN/TRUSTEE. 

DO NOT TRY TO DELIVER THIS FORM IN PERSON. IF YOU DELIVER IT IN PERSON YOU MAY BE ASKED QUESTIONS THAT YOU ARE UNABLE TO ANSWER AND YOUR REQUEST WILL BE DENIED. HOWEVER, IF YOU MAIL CORRECTLY COMPLETED FORM YOUR CUSTODIAN WILL HAVE NO CHOICE BUT TO PROCESS THE REQUEST.

To obtain mailing address simply call the number on your statement and tell customer service that you would like to rollover/transfer your retirement account and ask for the address where to mail the form. Be sure to ask if Medallion Signature Guarantee is required.  Ask if request can be faxed. If so use this option to speedup the process.

This form has interactive fields; you can complete it on your computer and then print for signature

FIELDS INSTRUCTION:

Type the name of your Solo 401k Trust at the top of the form.

Section A:  PARTICIPANT INFORMATION Please fill in your full name, phone number and Social Security Number.

Section B:  ACCOUNT TO TRANSFER/ROLLOVER

Custodian Name:  The name of the financial institution currently holding the IRA you wish to transfer.

Account No:  Your account number at the financial institution currently holding the IRA you wish to transfer. If you are transferring multiple accounts, please complete a separate form for each account.

Custodian Address:  The address of the financial institution named above where you will mail your transfer request to. Please call your financial institution for this information if you are not sure of the address.

Contact Name:  The name of your representative (if you have one) at the above named financial institution.

Phone No:  The phone number of your above named Contact or the financial institution’s Transfers Department.

Section C:  CASH TO TRANSFER/ROLLOVER

TO TRANSFER CASH:

You MUST provide your financial institution with liquidation instructions prior to submitting this request or delays will occur. All assets must be in a cash position ready to transfer. If you wish to transfer cash, select one of the following options:

• Transfer ALL available cash – choose this option to transfer all available cash from your account

• Transfer EXACTLY – choose this option to transfer an exact amount

Section D:  DELIVERY INSTRUCTIONS FOR CUSTODIAN/TRUSTEE

Mail:  Select this option if you would like your financial institution to mail the check to your Solo 401k Trust.  If you select this option you can open your Solo 401k checking account after you receive transfer check.

Make checks payable to:  ABC Ventures 401k Trust, FBO John Smith

Please use the format above and replace with your own info. You can find your exact 401k trust name in your Adoption Agreement.

Mail to: Type the address where you wish for the check to be mailed to. Normally, you will specify your address and then when you receive the check you will take it to the bank and deposit it into your new Solo 401k Checking account.

Wire:  Select this option if you would like your financial institution to wire funds to your Solo 401k checking account. Please be advised that there might be a fee for incoming wires assessed by the bank.  If you select this option you must open your Solo 401k checking account prior to submitting this form.

Institution Name:  Type the name of financial institution (bank or credit union)

Beneficiary Acct Name: ABC Ventures 401k Trust, FBO John Smith

Routing Transit No. (ABA) & Beneficiary Acct No: Your banker will assist you with this

FBO: John Smith

Section E:  PARTICIPANT AUTHORIZATION: Sign and Date

Section F: Signature & Acceptance by Solo 401k Plan Trustee

By: Print your name in the following format – John Smith, Trustee of ABC Ventures 401k Trust

Replace with your own info.  Sign and Date.

Note: Some but not all custodians require a Medallion Signature Guarantee.  This “medallion stamp” can usually be obtained at your bank.  Please be sure to ask your current custodian if they require this type of signature guarantee to avoid unnecessary delays.
